The league our teams play in, Grand Valley Soccer Association (GVSA) is NOT changing the software program it uses to manage the league as we originally believed.

* MSYSA Medical Release
CONCUSSION DOCUMENTS - REQUIRED
* MSYSA Memo
* MSYSA Parent/Player Information
* MSYSA Parent/Player Medical Clearance to Play
Please bring all completed forms (DO NOT DATE any of the forms) to tryouts.
Verify with your onsite coordinator on procedure to sign up.
For Select Coaches and Team Managers:
* Risk Management Information
* CONCUSSION AWARENESS - REQUIRED
All coaches, employees, volunteers, and other adults involved with a youth athletic activity MUST complete a concussion awareness online training program. This only needs to be
completed once by each adult member. For convenience, the "Heads-Up Concussion
Training" will be integrated into individual Got Soccer accounts when a new background check is submitted.
Every adult member is responsible for his/her compliance with the online training.
